Who needs one

The health certificate is required by the Department of Health for people working in activities with direct public contact where a communicable disease could spread: food handling, restaurants, school cafeterias, barbershops and salons, childcare, elder care, and much of the healthcare sector.

Not every job asks for it. When a posting mentions it, it is almost always a requirement to start, not to apply. But having a current certificate gets you working weeks earlier than someone who starts the process after the call.

What it includes and where to get it

The certificate is issued by a physician licensed in Puerto Rico and includes a physical exam plus lab tests that vary by job type. Food handling and healthcare roles usually add specific tests, including hepatitis in certain cases. The lab returns the results and the physician signs the certificate.

You can get it at Department of Health offices, community health centers and private clinics. Costs and turnaround vary by location, so call ahead: asking about price and whether they take walk-ins saves you a trip.

Validity and renewal

In most cases the certificate is annual. Put the expiration date in your phone calendar with a two-week reminder: an expired certificate can pull you off the schedule until you renew, and in kitchens and healthcare facilities that happens more often than you would think.

The certificate belongs to you, not the employer. If you change jobs, the same valid certificate works as long as it has not expired and the type of activity is the same.

Does the employer have to pay for it?

The law does not generally require the employer to pay, though many reimburse it or have arrangements with clinics. Ask in the interview; it is a normal question, and the answer tells you a lot about how they treat staff.

Is it the same as a police record certificate?

No, they are two different documents from different agencies. The health certificate is signed by a physician under the Department of Health; the criminal record certificate is issued by the Puerto Rico Police. Many jobs ask for both.
