Summary: The Care Team Navigator (CTN) acts as the primary point of contact between the patient living with dementia (PLWD), their caregiver (referred to as the dyad), and the care team at the Baystate Memory Assessment and Care Center (MACC). Much of this work is by video or telephone appointments. The CTN role does not require a college degree or background experience in health care. Excellent communication skills are a must.
